Role Description

This project will develop a comprehensive, evidence-based business case for PBF Australia’s Hospital to Home Peer Support Program, supporting individuals transitioning from hospital following spinal cord injury. The program is designed to improve outcomes, strengthen independence, and enhance community reintegration through structured peer support delivered by people with lived experience.
The role will involve researching the need for post-discharge peer support, analysing current service gaps, and designing a scalable, sustainable model aligned with PBF Australia’s strategic direction. This work will inform the development of a high-impact program supporting individuals at a critical stage of recovery and adjustment. Key activities include: • Researching peer support models and post-discharge needs • Analysing service gaps and identifying opportunities for improved support • Designing program models across in-hospital, community, and virtual delivery • Exploring in-house, partnership, and hybrid delivery approaches • Developing staffing structures, workforce requirements, and training needs • Preparing costings and financial modelling • Aligning with organisational strategy and relevant health and disability frameworks • Contributing to a comprehensive, fundable business case The role will engage with PBF Australia leadership and program teams, people with lived experience, healthcare and rehabilitation providers, and external stakeholders and potential partners.
